First Visit

At your first visit to our office, you will be introduced to our office team and be asked to fill out a few forms to provide us with a complete medical and dental history. 

During the initial consultation visit, our oral and maxillofacial surgeon will ask the patient about any symptoms that they are experiencing and perform a thorough clinical examination. As needed, diagnostic films may be taken at this time. Based upon a review of the patient’s medical and dental histories along with the exam findings, our oral and maxillofacial surgeon will make the appropriate recommendations for care. Depending upon the results of the consultation and the nature of the specific case, a procedure may be performed at this visit or a subsequent scheduled appointment. Typically, a complex medical history, the need for further diagnostic testing, and specific types of oral and maxillofacial surgical procedures indicate the need for a second appointment and possibly additional visits. 

To provide thorough and efficient care, we ask you to please bring the following information to your first visit:

  • Your surgical referral slip and any diagnostic films. Your dentist or referring provider can also send us this information electronically in advance of your visit. 
  • A complete list of any medications you are presently taking along with contact information for your physicians. Also, inform our office if you are pregnant or think you are pregnant. We also need to know if you have any chronic or acute health conditions and if you have a medical prosthesis such as heart valves or artificial joint replacement.
  • If you have medical or dental insurance, bring the necessary completed forms. It saves time and allows us to help you submit any claims.

We also ask patients to adhere to these additional guidelines and requirements for care:

  • All patients under the age of 18 years must be accompanied by a parent or guardian to their initial consultation visit and as mandated by our office for any appointments after that. 
  • Patients undergoing IV anesthesia for surgery must have a preoperative consultation and physical exam. In some instances, a medical clearance may be required from the treating physician before a surgical procedure is performed. 
  • Patients receiving IV anesthesia for surgery must follow all preoperative guidelines, including adhering to the NPO (nothing by mouth) instructions after midnight before the procedure.
  • An adult escort must remain at the office during the surgery and is required to accompany the patient home after the procedure.
